Dec. 16th 1:30-2:30 pm: Dante met with the entire PT Team with the exception of Julyeishka who was busy with a client off-site.
At the meeting, Dante thanked and reminded folks to complete their monthly mental health screenings. 3 of the 6 participated and 1 person (Sandra) asked to redo hers after Dante reaffirmed the surveys are anonymous.
Next, Dante pulled up the recently tweaked PT workplan redone last month with leadership. Dante's goal was to discuss Action Steps for each of the goals and make sure they align with what the team feels is realistic and achievable. All of the Action Steps have now been revised to match the tweaked goals.
Here is a list of the next steps outlined for each goal:
Goal 2: CHC, Inc. will ensure RW patients achieve an 85% HIV viral suppression rate by the end of the reporting period. Develop a palm card campaign for New London patients, highlighting the benefits of viral suppression, how to achieve it, and what options open up for patients who are virally suppressed (e.g. U=U, LA injectables, etc.).
Goal 3: CHC, Inc. will ensure syphilis testing rates increase from 62% to 75% by the end of the reporting period. Have a discussion with Dr. Haddad on developing a way to ensure providers are requesting syphilis tests when prompted to on the EMR. Perhaps scheduling a training or strategy to remind providers.
Goal 4: CHC, Inc. will improve the percentage of dental appointments from 21% to 50% for RW patients. Similar to Goal 2- develop a palm card campaign promoting oral health for patients. MCMs agreed to continue scheduling out patients as far as they needed to, to get patients seen. Then once they're seen, they can book follow-ups 6 months in advance.
Goal 5: CHC Inc. will focus on mental health support to the Case Management team. Continue participating in mental health surveys each month.
Goal 6: CHC, Inc. will increase PrEP Prescriptions 20% by the end of the reporting period. We're going to start with a provider detailing training for providers not linking their HIV negative patients to PrEP. The outreach team mentioned going to the same shelter every Tuesday and not seeing as many clients for HIV testing. Dante discussed finding additional locations to go to so instead of the same shelter every Tuesday, alternating with a soup kitchen (or other determined location) every other week instead. Prevention team seemed to like this idea.
